Delaware State University and the Center for Disability Resources (CDR) value all aspects of diversity and ensure that any University employee with a disability receives equal and equitable access to employment, programs, goods, and services. CDR works proactively with employees and University stakeholders to gather information and determine reasonable accommodations.
Accommodation Request Process — University Employees
- Complete and submit the online Employee Request for Accommodations form. Select "Employee" for your role.
- Submit medical documentation that meets the Documentation Requirements. Documentation can be uploaded with the form or to your Accommodate Profile. Medical information is housed securely in CDR, is not included in any employee file, and is not shared without the employee's written consent (except a verifiable need-to-know).
- Engage in the interactive process. Once documentation and the request form are received, the employee is emailed to schedule a meeting with the Director of CDR for a collaborative conversation about barriers and requested accommodations.
- Accommodation determination. The Director reviews all documentation and the interactive process to determine reasonable accommodations, reaching out to stakeholders only as needed; no confidential medical information is shared.
- Accommodation letter delivery. If approved, the Director issues an Accommodation Letter to the employee, with a copy to the supervisor and any party responsible for facilitating the accommodation.
- Accommodation letter renewal. The letter is generally valid for one semester. To renew, log in to Accommodate, select "Accommodation" then "Renew Accommodation Letter," click "Add New," and follow the prompts.
Employees are expected to maintain contact with CDR and remain actively involved in the accommodation process. Direct questions or concerns to the Director as soon as possible.
